Learning Objectives: 

--Participants will learn about homelessness in Chicago and the scope of this health-related social need within hospitals.

--Participants will learn how the nationally-validated "Housing First" model produces better outcomes and substantially reduces healthcare cost & utilization.

--Participants will learn how to partner with CHH and Managed Care organizations to identify and place at-risk chronically homeless persons into permanent supportive housing.
